You can combine several pdfs together using Acrobat Exchange, without having
to get Compose. However, any links from one pdf to the other will have to be
manually redone in Exchange once you've combined the pdfs.

In Exchange 3.0 (or Acrobat 4.0) use the Document-Insert Pages command.

If you want to avoid the "huge honkin' Word doc" altogether, get an add-on
product for Acrobat, such as Compose. You can then create each .pdf
separately and combine them into one .pdf (using any of several methods).
Compose will appear as a toolbar in Acrobat and allow you to link your TOC
and create thumbnails. Compose is a bit pricey though.
