I'm guessing wildly about procedure here, because
I'm using Word 97 at work.

Try switching to Header and Footer view and using
the Header and Footer toolbar buttons to verify the
format of your page number and to insert a page
number (in addition to the "bad" one you already
have) to make sure your selections are, in fact,
making it into the footer. Do the two have the same
prefix?

In the Page Number Format dialog, make sure you have
the "Chapter starts with style" value set correctly.

Make sure you really do have a Heading 1 in the
document and only one Heading 1 and that it is
autonumbered with an Outline Numbered type number
string.

Do you use the chapter number as a prefix for figure
and table numbers? Try inserting a dummy caption.
When you get to the Caption dialog, click the
Numbering button and note the settings specified for
the format there. This is the same dialog as used
for the page number format, but the Caption route
has the advantage of giving you a preview of the
result.

Finally, make sure you're not getting confused by a
multiplicity of sections, each with their own
headers and footers. Make a trash copy of the
chapter and delete all the section breaks (if there
are any). See if you can get it working a single
section.
