The only way I found to maintain an underline to the base word is to insert
the raised or superscript number in an shrink-wrapped equation. Insert the
equation and adjust for You can underline the whole word or phrase. Just
make certain that your equation font and size matches the rest of the text.
Perhaps you could insert a superscript character to assist in size and
alignment.
