All -- FYI ... I recently indexed an excellent style manual, which I
believe will be published in December 2000. Here's the info on it:

Science and Technical Writing: A Manual of Style, Second Edition,

Of course, I thought the indexing guidelines, which I reviewed for the
author, were quite good. :-)
