According to Donald A Norman's book "The Design of Everyday Things," pasat
serious problems in airline cockpits and nuclear power plant control rooms
that have been attributed to "human error" (of presumably well-trained
people) could have been alleviated with better design.
Training may help people use bad design better, but the bd design still
increases the chances of error.
