Correct'g Publishd Docmnt'n - Procedure?

Does anyone have any good ideas on procedures to
follow when correcting documentation that is already
published and sent to clients?

At my last job we had a procedure (a Change Control
log) but no one ever used it. We were also supposed to
re-line changes in the new version, which we did.
Thing salways fell through the cracks, however. At
this co, I wanted to do something else - something
that made better sense and worked.

I have heard of version control software but I don't
know which ones are best. Also, it seems we need more
than just that; I think we need a procedure. 

Any ideas on an administrative procedure would be
appreciated.
