Search for "Karen Elizabeth Gordon" in the Books
category at www.amazon.com, and pick just about any of
her books. The best are:

These books are as much fun to read as they are
helpful.

Get a book on indexing, too. A weak index cuts the
usefulness of your documentation. I have Larry Bonura's
"The Art of Indexing" (ISBN: 0471014494), also available
at www.amazon.com.
