David Castro wonders <<Would you typically create an index for a document
that is used for instructor-led training classes?>>

Depends on whether you mean resource materials for the teacher, or the
student handout. Speaking as a student, I often refer back to old training
handouts, and find it really annoying when there's no index and I have to
skim the entire book to find something. Speaking as an occasional teacher, I
don't think the index would be essential (since I should know the manual
inside out before giving the course), but it does prove helpful for those
momentary brain-blanks when I need to look something up but can't remember
where it's hidden in the manual.

"Technical writing... requires understanding the audience, understanding
what activities the user wants to accomplish, and translating the often
idiosyncratic and unplanned design into something that appears to make
sense."--Donald Norman, The Invisible Computer
