Resources for flowcharting

Can anyone point me to books or online resources about how to create
flowcharts?

I've done many but am not sure if they're as clear as they could be.
For example, should you limit the entry and exit points or use a different
side for each entry or exit point?
How effective are off page jumps?
Etc.

I searched the list, and the question was posed, but not answered.
