Just click on the little paragraph-mark tool button (usually near the upper right of the Word window).

No need to select all first--Alt+F9 toggles field codes for the entire document.
(Shift+F9 toggles display for the selected fields only--maybe you were thinking of that.)

When hidden text is visible, you can search for ^d XE (that a caret, a lowercase "d," a space, then an uppercase "X" and "E") to find index entries. Don't search for hidden text, though--for some reason, Word doesn't think of index entries as hidden, even though they are.
