Has anyone out there ever designed a questionnaire?

I work for a company that produces messaging software for Internet service providers. We have a detailed questionnaire that we send to new customers, which covers topics such as their current mail service, their business goals, and their existing technology. We (the technical publications department) have been asked to revise this questionnaire. Unfortunately none of us has much experience designing anything other than procedural manuals. Does anyone out there have a sample of a well-designed questionnaire that they can send me? I would really appreciate it.
