Where'd you get that idea? Seems a rather broad statement to make. I can say
for sure that that's not the way it works everywhere. Maybe in a rocket
science type environment, but in my environment (documenting Web
applications in which I was involved from the ground floor), the programmer
didn't ever even look at my document/help, let alone ensure its adequacy and
completeness. That was my job.

It seems the type you're referring to are the type so often complained about
on this list--the "pretty formatters" with no technical know-how.
