Consider the variation of ForeHelp (Pro? Premier? can't recall the name) that does Help in various HTML formats, including Microsoft's HTML Help. http://www.forehelp.com/

ForeHelp isn't based on MS Word, but you can export to RTF and put the results in Word or any other WP/DTP program to produce paper docs from the source.
