If you're looking to do a simple multiple-choice type form, it really
couldn't be more simple. There really isn't that much Perl code needed to
get the answers in, parse them and print a page that gives feedback. (It
wouldn't be hard to mail it either--if you're using a Unix based server.)
If you can't find anything that will suit your needs, I can write a
boilerplate form, script and instructions on how to edit it (I'd be doing
it on my "leisure" time, so it may take a couple of days to get to it). If
anyone else is interested in something like this, I can post the
boilerplate test form on my server.
