Well, for lack of a real "discovery" solution (i.e. one that figures out what the origin of the problem itself), I took a less expert friend's advice as follows:

First reboot your computer to clean out everything. Probably not needed, but what the hell?
1. In Explorer, Rename current document to something other than current name.
2. Make new, empty file and attach your template to it.
3. Insert (NOT copy) your old, renamed document into your new document.
4. Save the new document under previous name.
5. Insert your table of contents again.

so did that work? I would have said: Copy everything but the last paragraph mark of the original document and paste it into a new one. Insert File doesn't always leave the nasties behind, because they are contained IN that last para mark.

You need to turn on Hide/Show for this. It works for all manner of Word Weirdness.
