While I was researching Human-Computer Interaction and similar topics, I came across a lot of anecdotal stories about accidents, death and major losses encurred due to bad communication in general. One example I do remember was talking about internationalization, and the use of the phrase "Pull Up" in a cockpit navigation system. If I remember correctly, the pilots did not speak english and didn't understand what the warning meant, and so slammed their plane into a mountain (by accident). I believe the confusion arose because the phrase typically used to "Pull Up" is "Climb." Unfortunately, I don't remember where I read that.

My suggestion is to do a search on google about Human-Computer Interaction or human-centered design. You should come across many stories of the kind you're looking for.
