We call that a Project Plan, and it include everything
you listed, and maybe additional topics.

Some suggested additional sections in the Project Plan
might include proposed schedule, deliverables,
what this project INCLUDES and DOES NOT INCLUDE
(use whatever words sound appropriate), etc.
We title the section IS and IS NOT.

The Is/Is Not section is really valuable to keep
various people from assuming that XYZ is included
in the project, when in reality it IS NOT. Also
handy to prevent Scope Creep. And this is not that
weird person down the hall with binoculars! Set
limits as to what is included in the project and
once set, DO NOT exceed them, unless everyone has
bought off on the change in scope, including getting
more time and money.

Scope is usually a portion of the Project Plan.

The written scope statement identifies both the
project deliverables and project objectives. It
provides a basis for confirming or developing
common understanding among the stakeholders.
