I'm working on something very similar at the moment. I hope to have it complete in the next few weeks - if you can wait that long. It may not be precisely what you are looking for, as one of the requirements for my method is html written for the express purpose of converting to xml - but you can still take a look when I'm finished.

Just so you know, I'm converting from Word(purpose built template) to html using HDK(purpose built template), and then using Dreamweaver(purpose built template)/Perl to manipulate the HDK output. From there I'll be using Perl to convert the html to xml. It may not be efficient, but it's what I know how to use. What I have not yet covered is DTD/Schema generation.
