Use Last name, First name.
In the US, many members of the same family do not share the same last
name, so "family name" is neither appropriate or accurate. For example,
there are three last names on the mailbox of my single-family home.
"Given" name is also not clear. We give ourselves all kind of names here.

But everybody knows what their *first* and *last* names are. Those who
have concocted new names for themselves, or who have hyphenated their
names with those of their spouses, have a separate set of issues to deal
with, but it comes up often enough that even they will know what to enter
in a field called Last Name.
