Interesting take you have on this movie. I could add that one reason
the US nuclear propulsion program has not had similar incidents is
the use of excellent written procedures, combined with lengthy
training.

A policy of "verbatim compliance" with written procedure, coupled
with extensive training and education of all personnel involved with
the propulsion plant, has resulted in an amazing safety record.

And to tie back in with recent threads, the procedure writers _are_
technical experts in the material ... ;)
