The headings are actually enclosed in bookmarks. What's happening when the x-refs break is that your cursor is just outside the closing bit of the bookmark. I can't remember whether the bookmarks on headings show when you turn on the display of bookmarks (Tools > Options > View) or if there is something special you need to do to see them.
