Hi James,

I typically use the following structure (not a standard format, just
what I have used):

-------------- Error Matrix --------------
ID --- Message --- Description --- Resolution

This is usually wrapped in a Troubleshooting chapter that also includes
topics like:
* Frequently Asked Questions
* Installation Issues
* Configuration Issues
* Run time Issues
* /Application/ Issues

And so on.
