A form that has boxes for Mrs./Miss/Ms. hardly forces women to choose
anything than their preferred salutation. Believe it or not, there are
still otherwise modern women in the world who subscribe to the
"old-school" notion of distinguishing between married and unmarried
status. In my opinion, it is totally ridiculous to assume that all women
would prefer to use "Ms." or any other salutation to the exclusion of all
others. That some forms provide for all salutatory options merely reflects
the (perhaps unconscious) desire of the forms designer to appeal to the
largest audience possible.
