Here's what I need to do: create an online survey for a non-profit organisation. We expect as many as 200 responses and will have perhaps 50 questions. It will be a one-off (well, maybe repeated once a year). Needs to be cheap (under US$100), easy to set up, reasonably customisable, provide responses as a spreadsheet (at least), and not be cluttered with ads.

I've googled and found lots of online survey tools, but of the 50 or so I checked, only two are cheap enough and appear to offer the needed services. (The free services I've found are too limited in the number of questions allowed, or the number of responses reported, or both.) The two I've found that meet my criteria are SurveyKey.com (their Pro Plan) and the upgrade version of free-online-surveys.co.uk (those two services price out very much the same for what we want), but I didn't have the energy to search through thousands of possibles. Most of the ones I checked were more in the US$500 range.

My question to you is: have you used an online survey tool that meets these requirements, and if so, did it work well and deliver what you needed?
