Never, EVER, confuse those two terms. They are
completely different. A manager *can* be a leader, but
a leader certainly doesn't need to be a manager.

No, they aren't, by nature. Unfortunately, they are
*by practice*. You should try to avoid subjectivity in
performance appraising as best you can. Otherwise YOUR
appraisal can be rebutted/attacked subjectively. No,
you need defined requirements with which to weigh your
employee's performance against.
