Yes, Word considers that a 'form' in a 'protected document.' Word provides a
GUI for creating these forms; you don't necessarily need to create fields by
hand. See Word's online help.

Watch out for selection bias and Heisenberg effects... i.e., do you want to
create a bias toward:
- people who have or know how to install PDF readers?
- people who know how to fill out Word forms?
- people who have web access and know how to fill out Web forms?
Does your survey need to ba anonymous?
- If you make it anonymous, people can fill it out multiple times
(potentially).
- If it's not anonymous, your survey will be filtering out people who
prefer not to be identified.
