InDesign has excellent XML support (I've just started experimenting
with it).

This might not be what you're looking for, but InDesign exports
directly to PDF with support for hyperlinks and bookmarks. Between
InDesign and Adobe Acrobat, you can create a robust help file in PDF
form. Using Acrobat 5 or Acrobat 6 Pro, you can also add buttons, etc.
to the pages if you want.
