For desktop PDF creation, you may want to check out activePDF Composer.
Composer generates "Intelligent PDF" from Microsoft Word, so headings are
converted to bookmarks by default. Additionally, hyperlinks are retained,
comments are converted to sticky notes, etc.

If you're looking to set up a solution on the server, try the activePDF
DocConverter/Server bundle. You can set up "watched folders" that use
predefined document profiles to apply PDF settings. DocConverter can be
used in batch mode, and it also supports "Intelligent PDF" from MS Office.

Good luck!
