For the PDF output I create with Word, I've been using a nifty product for several years. Sonar Bookends Activate does a lot more things, now; but what it's always done is add hyperlinks to all the page numbers in the index of a PDF document.

It was about $200 when I first registered, which seems like a lot--but it has saved me days of tedious and error-prone drudgery.

For the record, I have no financial interest in Sonar Bookends Activate or its maker, Virginia Systems (www.virginiasystems.com). They make lots of other products that I probably wouldn't use, but this one has provided great return on investment.
