This is a guess. You had one of your network printers selected as your default printer when you opened the Word doc. You did or didn't update the TOC links, and everything looked hunky-dory. Then you clicked the Create PDF button and Word said it had to save the doc before proceeding. So you clicked OK, and then you proceeded with creating the PDF.

Only what Word did at this point was repaginate the document--because you changed from your network printer to Distiller--without bothering to update the TOC links.

I'm not sure this is exactly what happened, but just for grins, try setting Distiller as your default printer (Start > Settings > Printers ... Right-click Distiller and choose Set as Default from the context menu) before starting Word. Then open your doc in Word and update the TOC before creating the PDF. See if that solves the problem for you.
