I spent six years as an aircraft electronics technician (avionics) and
often appreciated how things had been designed so that they simply could
NOT be installed or connected incorrectly.
In any system involving human beings, mistakes will be made. For an
accident to occur, you need two things: a mistake and a system that
allows the mistake to proliferate. The design should have been such that
the switches could only be installed one way (a keyway) and then it
should have been tested * at least once. I don't think it has much to
do with TWs though.
