Dick,
This is a bit of a shot in the dark, but the American Medical
Association publishes its own style guide, and they might cover this
topic therein. Perhaps someone on the list has (or has access to) a
copy and can check.
As for a hierarchy of prestige, I can offer just one data point: in
situations where the subject has both an M.D. and a Ph.D, I have
nearly always seen the M.D. listed first, like so: Dr. Joe Blow, M.D.,
Ph.D.
Having said all that, in the absence of a definitive ruling I would
vote for chronological order.
