Number (4) below makes the TOC live, but while you have the "change
conversion settings" open, also go to the last tab called "bookmarks" and
choose the styles you made to denote your headings, subheadings, etc. As
you've been able to create a proper TOC, they already should be fine to
produce properly indented bookmarks.

If you want the bookmarks to show automatically when the file opens, set
that in Acrobat.

(1) Open up Microsoft Word. If Word and Acrobat are installed on your
machine correctly, there should be an "Adobe PDF" menu on your Word menubar.
(2) Select "Change Conversion Settings" from the "Adobe PDF" menu entry.
(3) Click the "Word" tab on the resulting "Acrobat PDFMaker" dialog box.
(4) Make sure that "Convert cross-references and table of contents to links"
is checked.
