Most of my work has been with the US Government, and the agencies I worked  
wanted to look like the others.
 
So, that is how I have recently done things.
 
The Government Publications Office (GPO) set most of the standards, for  some 
organizations, and Yes, "decimal" headings were required.
 
"When you only have a hammer, everything is a nail."
 
I did not mean to say that I liked it, it was just easier to please about  
400 people with a 'standard' for their documentation.
 
BTW, I would not want to try to attempt to promote any 'publication design'  
ideas with the 'entrenched' people I had to work with.  Some can't even  agree 
on when to use - will, would, should, shall, may, can, and the  like. 
 
Especially working in the DC area.
