Two different issues here.
First, as an official "credit", not, I don't use my name. The problem
is that if I leave and the content says "For isues regarding this
document, see John Posada.", they won't know who to contact in my
place. I always refer by department or job function.
OTOH, so that you can prove you write it...many of the things I
document include either status bars, "signed in as", etc. I don't go
out of my way to remove those attributes. Makes it easier when you
include it in a portfolio and they want to be more assured that I
wrote it.
