We found that a 3-tier approach works for us. Tier-1 (when you don't have much time/money), just change product and company names (messy) and the front page. Tier-2: also change the look and feel - fonts, styles, notes, headers/footers, drawings etc. Tier-3: also map the contents into our standard outlines and fill any gaps. From this point on, ask only for raw material of future versions and manage the doc as if you wrote it yourself. Move from tier-1 to 2 and then to 3 if there is business justification.
