Odd FM behaviour

We have something odd in the numbering of figures in a FM doc and
wondered if anyone had come across anything similar or had any
suggestions. Probably something very simple but I can't see it at the
moment.
Book uses same tags etc throughout - set template - and what we have is:
C1 has figs 1 - 5, all OK
C2 has one fig which it has as Fig 1.
Adding a dummy to C3 and it puts it in as fig 7! So it's obviously
counting the fig in C2, just not displaying it properly. As I said,
we've checked the tags and they're the same in all the chapters. Small
doc so we can do it manually but I'd like to know what's causing it.
