Documentation on Demand?

My firm has a set of standard products which of course have complete manuals.
Additionally, we do quite a lot of projects with customised hardware and software, which also get
documented.

How do you handle this situation? Do you provide an extra document that includes additions and
specifies the deletions? Do you customise the whole documentation including all additions, changes
and deletions? Or do you have any other suggestions?

The documentation is for now print-only.
