Cratig DOcumentation for Custom Builds of an Application

Hey Whirlers,

  I may have read some reponses to something very
similar to this, but I'm not sure if I'm just making
stuff up. 

  I have to create best practices for documenting a
custom-built application. Are there any
resources--besides you geniuses, that is ;)--for going
about this sort of thing?
