Try going from Visio to WMF, EPS, or another vector format first. My
guess is that the ouptut settings for creating PDF are messing with
the screen-aligned vector placement.

Yes, that is how I first noticed it, in print from pdf. But it's also visible on the pdf on screen if you look closely.

Grouping the background image with its arrows does no good.
