FM 5.5 TOCs

Can anyone tell me how to (1) get the word "Contents" to remain on 
the body page of a TOC, without being overwritten when the TOC is 
regen'd and (2) how to get the body page of a TOC to pull in the page 
numbers?  I've got (I think) the variables on the reference page 
properly defined, but when I generate the TOC, the body page pulls in 
only the tagged paragraph, not the accompanying page numbers.

I can't seem to find anything this specific in the old FM help.
