The bookmarks are indented in the Acrobat bookmark pane? Just select the indented bookmarks and drag them out.


Also some of the bookmarks have the chapter number (eg, 2.4.1), and
some don't. This seems often but not always related to having an
alphanumeric string at the start of the chapter title.

You can edit the bookmarks as well, if you wish.

If the document pages look as you expect them to and the only problem is with the bookmarks, I say fix the bookmarks and go on with your day. If you really want to know why this is happening, you'll have to dig into the Word file (initially) to look at how the headings are coded and then look at the AuthorIT file to see what artifacts are causing the problem. But if it's only a few documents I wouldn't dig that deep myself.
