We never confuse the field name with the item in question. Thus, we'd write:

Enter your last name in the Last Name box.

not

Enter your Last Name.

We keep the procedural steps as concise as possible. If the field requires definition we do that either in the preamble leading up to the steps, or in a table that could precede or follow the steps.
