How about a macro that adds the user's name to the document when s/he
prints it?

You're assuming that they printed it. More often it's a fourth or
fifth-generation "illegal" photocopy of an old document that's been replaced
by a new controlled version.
I have resorted to posting a big sign taped to the top of my computer, which
I flip over to cover the screen every evening:
"IF YOU LEAVE SOMETHING ON MY DESK WITHOUT INDICATING WHO LEFT IT, IT WILL
BE THROWN OUT."
There was another orphan on my desk this morning. I threw it out.
Eventually they will Get It.
Our procedure -- in which everybody has been trained -- tells them to fill
out and attach a documentation work order sheet. My boss has given me
permission to post my sign and to follow through on the threat.
