I don't really have a problem with your original form, but then I'm no
expert at this! :-)
One alternative I can think of is to use a sentence or sentence
fragment to introduce your bulleted list, so the need for "or" no
longer exists. Something like this:

Choose either of these actions:
- Enter a name for your personal clown.
- Leave the field blank for us to choose one for you.

Yes, it's not a great idea, and somewhat stilted and formal, but it
removes the need for "or."
