Mark's right, at least about Merriam-Webster; "and/or" is listed in the Eleventh Edition at my elbow.

That's an entirely different "and/or" problem.
You searched electronically, and the slash was
understood as a delimiter.  But "and/or" is in
all those dictionaries.  You may have better
luck sneaking up on them indirectly through
www.onelook.com.
