Saying someone "has a suffix to their name" sounds like the saying
"hasn't a penny to his name". It definitely trips me up.
How about "when searching for a person with a suffix appended to their
name"? or "when searching for someone whose name contains a suffix"?
Both sound better to me.

I'd go with "to." Words have suffixes and prefixes and I wouldn't refer
to
a prefix being "in" a word, but a prefix "to" a word. A suffix to a
word or
a suffix to a name should be refered the same way as a prefix.
