It has had follow ups in both places, like a mutant strain of
cross-posting. The offlist portion was unusual in its own way, being
cc'd to list subscribers and non-subscribers

I haven't notice the need for CC etiquette before now, but I think this
thread portends two points.

1. Requests not to be copied on an offlist thread are reasonable.
Consider how difficult that is to comply with before giving that task to
everyone in a long CC list on unsolicited emails.

2. If you must send list-oriented mail to non-subscribers and
subscribers, use BCC or an alias to protect the privacy of list members.
"Presumptuous" may not be a strong enough word to describe anyone
sending list members' email addresses to non-subscribers without
permission.

A third point about CC comes up from time to time:

When posting to the list, sending a CC to the poster(s) whose text
you've quoted is a courtesy, but not a requirement. No need to CC anyone
else in the thread without a good reason.
