FrameMaker 8: Table numbering reset after text inset?

I have a chapter with 15 tables in it, all of which number perfectly
except for table 15, which for some reason thinks it's table 1.

The table's title uses the same tag as all the other tables. None of
the paragraph tags between the last correctly numbered table and the
incorrectly numbered one use an autonumber scheme with the same label.

There is a text inset between table 3-14 (correctly numbered) and 3-15
(incorrectly numbered as 3-1).

All I can think is that the text inset has messed up the numbering,
but I don't know how.

Has anyone else run into this? If so, how did you solve it?
