I'll vouch for the Articulate suite and its Quizmaker product. I've been
using Articulate for about a year and a half and like it very much. Not only
am I creating some really exceptional CBTs, I'm also having a lot of fun
doing it.

And, if you just need to create a quiz, there is a 30 free download of the
entire suite.
